HOW TO: For first time user: 1.Click the Form Assistant button on the top right of the page which will show "Have no any information saved yet." 2.Fill the form normally . 3.Click the"Look for Appointment" button .A dialogue will jump out asking whether user wish to save the registration info or not. 4.Once the registration info been saved .user may find it from saved list by click Form Assistant button.The saved info displayed as that person's "surname:date" of birth 5.Click the Name from the list using saved info. 6.Click red "X" on each person to remove that person from the list. For saved info : Any change or updated of saved info will auto detected when user submit the form ,the dialogue will jump out asking user whether user wish to update or not.
